I was reading about El Infierno on Netflix and noticed its rated NC17, looks like its for extreme violence too. I thought about watching it but its three hours long.
It's well worth your time, it is one of my favorite movies I watched this year

A nastier piece of pulp fiction you will never find but also the most amazing black and white cinematography I think I've ever seen.
Done in 1999 by Les Bernstien the same year he did the visual effects and was the director of photography for Fight Club.

Rampage (1987)
I don't think it made it to DVD, but I remember seeing it on HBO a long time ago, and it's been on Netflix.

The Man Who Would Be King might not qualify as super obscure as it won a few awards back in the day but its Connery, Caine, and Kingsley in a faithful adaptation of Kipling's short story.
