When Bruce Willis realizes he is a ghost in "The Sixth Sense" (I saw this in the movie theater the day it came out before everyone knew about the twist)
When it is revealed that "the village" was actually in modern day times. I knew there was going to be an M. Night Shymalan twist but wasn't expecting that to be it.
When Donnie has a heart attack in The Big Lebowski.
When Jenny drops on Forrest Gump that the kid is his (still don't believe it and don't see how the kid doesn't have AIDS).

Sixth Sense reveal gotta be in there. More than one scene in Jacob's Ladder too, basically the same story hook along with the REALLY disturbing dance scene.
