These two are a bit borderline since they fell apart around 30-31, but Chuck Knoblach and Dan Uggla for a couple of Mlb 2b.

Denny McLain 31 wins in 1968 and out of baseball 4 years later at the age of 29. He was always in trouble during and after baseball - drug trafficking, carrying a gun on the team plane, embezzlement, and racketeering.

Grant hill practically is the mold. Him and Ken Griffey (although Griffey’s decline is somewhat relative).
