It's not necessarily weird, but just extremely uncommon. Commotio cordis requires several things to happen at once. The hit causes VFib, which is an arrythmia, which causes Hamlin to collapse. If they used an AED on the field, it at least means that they had a shockable rhythm. I didn't see the CPR, but they likely went through the algorithm several times until they had ROSC, or return of spontaneous circulation. Despite this, Hamlin likely has cerebral hypoxia due to lack of blood flow, which injured the respiratory centers in the medulla and the pons, which might explain the report that he had a pulse but was not breathing.
