There’s a first half play where Watkins is supposed to block the safety on the edge and pulls up and gets smacked by the safety. Thankfully he’s smacked so hard Dillon Jones is able to get by. But he definitely pulls up and receives the blow rather than actually block.
ETA consistency and effort are definitely areas he needs to improve.

he doesn't play hard. That's worse than the drops. He's talented but he'll be forgotten soon if he doesn't get his head right.

Leavitt not seeing the guy makes the ball placement make a lot more sense. The safety is flat footed and Leavitt could have easily thrown that ball over the top. But not seeing the safety, Leavitt just threw it to the space.
It's on both. WW runs towards the saftey when he could have either come back toward the LOS and into open space or cut up toward the endzone for Sam to throw deep. Sam messes up by not squaring his base and getting a stronger throw which could have been a TD but apparently didn't think he needed to because he never saw the safety.
Either way, it's a cardinal sin on a busted play to run into coverage and not either come back to the QB or find open space in the QB's field of view.
