Started a long time under tension program and immediately a hemorrhoid issue popped (literally) up. I’ve had them show up every once in a while, but this was immediately following the first workout that involved deadlifts. The next workout exacerbated the issue.

What are the options?

1. Stop lifting?
2. Treatment with OTC meds?
3. Surgery?

Inb4 stop getting anal blasted.

I don't have this issue, but I'll weigh in anyway.

1)Stop Lifting? Not an option really, in my opinion. Now...change methods? Sure.

2)OTC meds: I would think definitely, but I don't think they will do anything other than treat/control symptoms.

3)Surgery: Consult a physician first, no one else can really answer the question, but I'd imagine "Surgery as a last resort, only when there is no other option." would be the thinking. The typical post surgical issues for hemorrhoidectomy would have to be your main concern.

Really? Did you think he was planning to self perform the surgery?

I heard this was only a problem in lifters less than 5’7 due to center of gravity

This is where you can benefit from a vibrant locker room community and have someone pop it back in.

Preparation H for external shrinking.

Metamucil, daily with lots of water for internal binding of potential irritants passing through

Both have lessened symptoms enough to not notice the issue.

I stopped doing barbell deadlifts. Those caused one of the worst cases I've ever had.

Doctor recommended Metamucil for mine and I absolutely hate it, makes me feel like I’m leaking poop.

Went through this surgery just over a year ago. You're correct on the recovery. It sucked immensely. Granted, my doctor gave me that warning beforehand, but I had been dealing with the issue for over 5 years by that point.

Eventually it did come back and now I'll stick with softeners, laxatives, and lots of water. Not going to give up lifiting.

Someone on this board recommend rutin. I have been taking two every day for a few years and have not had an issue since.
