What exactly are the medals? Is that just something you get at higher ranks?

i think it comes when you max out a weapon or stuff like that. I want to say i earned on on the beta playing with just the m16A3. I unlocked everything for it and i think i got a medal. Maybe not

Medals come with ribbons
Earn 50 assault rifle ribbons, get a medal
earn 4 handgun kills a round, get a handgun ribbon. 50 ribbons, get a medal, Etc
etc.
Medals for guns require 5 service stars on that gun, I beleive

You got it exactly right. I've gotten medals for combat efficiency and the M16A4. Oh and yeah I've only got 17 hours in the game so yeah I'm the nerd.
