Have you ever passed out at the gym?

A guy at my gym was doing deadlifts, maybe a little over 300 on the bar, and just went limp after last rep.
Is it a breathing issue?
I had seen this happen on youtube before but never in person. We all checked on him but once I knew he was ok I couldn't stop laughing (he was too).
Has this happened to anybody on here? I think I have been light headed before but never full on passed out.

Never passed out, but definitely seen stars a few times after a tough lift, or if I wasn't feeling like my normal self in the gym.

I've never done it myself, but I've been close. When the music sounds like it's a walkman with dying batteries, that's my cue that I'm close.

I did doing 225 on a front squat. I took my deep breath, unracked, squat down and passed out. Apparently I blood choked myself.

I think I almost did on a rack pull. Had a deep breathe, my facet joint popped and I think I gasped or something and felt very faint. I got very hot and tingly and had to sit.

I hit a sumo DL PR several years back and got crazy light headed as soon as I completed the rep. Vision started to tunnel and got really dizzy, I had lay down for a bit.

Came close plenty of times on dl wherein saw stars etc. But front squat is the one that hasn't gotten me the closest.
Now 12 years ago after doing deads, presses and a couple other heavy lifts, I was warming up on the Smith machine with just the bar to do upright rows. No weight on the bar and felt fine, then suddenly I felt my back tighten and my legs went out from under me. I remember every breath cause crazy pain and I passed out. Woke up and they we're loading me into an ambulance. Turns out I had a nerve that got pinched and I passed out due to taking short breaths due to the pain.
Doc said surgery or pills, I didn't do either and after a couple chiropractor sessions I was fine. Now I usually have to go once or twice a year when I feel it tightening up. Nothing has ever come close to that first time though.
