I know of at least 2 people that have died because they were scared to seek medical care for fear of coronavirus and one more that's on a vent because he refused to accept his mundane diagnosis of pneumonia, insisted it was COVID-19 and did not get his prescriptions filled. Said if he wasn't getting hydroxychloroquine or chloroquine then nothing would work.

I guarantee more people die from acute onset dumbassery than do of COVID-19. People are going batshit crazy.

So the studies albeit small, and the anecdotal evidence of numerous patients saying it saved their life led him to the conclusion that he “thinks” it’s “unlikely” to see much effect from this drug? I wonder if he would take it if he were to get ill with Wuhan Coronavirus

There are dumbazz doctors also. There is a new experimental drug that is helping called Leronlimab. They are not sure how it works, but it has helped some critically ill patients get better.

The use of the drug has validity because of its use by doctors in the field. They didn't ask anyone if it works or could work, they prescribed it.

I get the feeling that some would prefer Plaquenil not work at all just so they can point to Trump and say you were wrong for being optimistic. The politics behind this part of it has been pretty shocking and embarrassing.

Here is one of my Derm thought leaders giving a summary/opinion of the newest French Plaquenil study:

In 80 in-patients receiving a combination of hydroxychloroquine and azithromycin we noted a clinical improvement in all but one 86-year-old patient who died, and one 74-year-old patient still in intensive care unit. A rapid fall of nasopharyngeal viral load tested by qPCR was noted, with 83% negative at Day 7, and 93% at Day8. Virus cultures from patient respiratory samples were negative in 97.5% patients at Day5. This allowed patients to rapidly discharge from highly contagious wards with a mean length of stay of five days. George Martin,MD commentary: This 80-patient study further supports the combined use of HCQ 200 mg TID + azithromycin 500 mg day on day 1 followed by 250 mg QD x 4 days. For patients with pneumonia and NEWS score=5, a broad-spectrum antibiotic (ceftriaxone) was added to hydroxychloroquine and azithromycin. The key to this therapy is early intervention. The mean time between the onset of symptoms and the initiation of treatment was 4.9 days and most patients were treated on the day of admission or on the day after (93.7%). The most important outcome measure is that the protocol allowed patients to rapidly discharge from highly contagious wards with a mean length of stay of five days. The author said it best: “We believe other teams should urgently evaluate this cost-effective therapeutic strategy, to both avoid the spread of the disease and treat patients as soon as possible before severe respiratory irreversible complications take hold”.

Plaquenil isn’t a terribly strong immunosuppressant compared to other DMARDs. I’ve never had a patients pcp or rheumatologist recommend stopping it before joints procedures and I’ve never required a patient to do so. There’s actually literature that shows patients have increased complications post operatively (infections, disease flare ups primarily) when some of these drugs are stopped pre-operatively. I don’t stop methotrexate either. Folks do fine.
