Robby Soave explains how its done in his book. I'll give you the short illustration with simple numbers:
Once elected president Obama assembles 10 close friends from his Chicago 'organizing' days.
He gets each of them appointed to executive (just show up) jobs at major corporations who want access to the President.
Each gets paid 1M a year = 10M a year total.
They kick back 10% 'thank you money' to Obama which = 1M a year to Obama...for doing zero work.
Wash, rinse repeat.

If they had a ghostwriter, the industry standard can be anywhere from 5 to 20 percent in terms of royalty, either net or gross. There can also be an agreement for a larger fee upfront, which can vary on how the ghostwriter wants to bill. Some ghostwriters have a one time fee based on the length of the book, some do it hourly, some do it by word, some itemize the book into categories, charging separate fees for the book proposal, research, travel, expenses, word count and rewriting.
Given the size of the advance, I would speculate that if they used a ghostwriter, PRH went with someone known and trusted in the ghostwriting world, and negotiated a hefty fee. It would have been considerably cheaper, though, to edit the book in-house rather than outright pay a ghostwriter. I would lean on the idea that since the advance was among the largest in history, and the book has made so much money already, that if PRH used a ghostwriter or ghostwriters, they would have found someone who was willing to take the upfront fee without a percentage of the royalties. But given the advance, I think it's more likely they had an in-house editorial team who helped the Obamas. The first two books of Barack's were similar enough in content and tone that I'm skeptical that a ghostwriter was involved, as the editorial teams in the 90's and 2000's were much larger.
