I'm of the mind that they've been in the "hospital"/astral plane this whole time. Last night's episode snuck in a repeated hint. When David came into Syd's room to snuggle the first shot of the room was from a camera angle under the bed. As the door is opened it shows no feet as David entered the room. The same thing happened in E1 but with the roles reversed, which at the time made me think that Syd was a figment of David's imagination. As far as can tell all of S1 so far has taken place on the astral plane.

I guess the diver from the ice cube is helping them? Slowly?
So strange. But do good.

From the previews, it looks like Flight of the Conchords guy is teaming up with Bill Irwin to help everyone. He can't leave the cube, so possibly he's directing Irwin?

I don't have an opinion on that.
How do we explain the "no feet from the under-the-bed camera angle"? They've now shown it twice so I think we have to consider it.

That's fine. I just know to completely ignore the opinion/analysis of anyone who thinks they were.
As for their feet, I just took it as interesting imagery. But I believe it happened in some form. I still think we are seeing events both as interpreted by and remembered by David, so there are some weird happenings surrounding what may be actual places/events. It's hard to know what particular details are real and not real in ANY scene in the show so far. But I still think we have a general idea of the sequence of events as I mentioned above.

No he was taking about Lenny. But pretty much everyone but David at this point is questioning reality and why they are there. David has been pacified by the monster/Lenny, who is very practiced and has been doing it to him for years.
Unless of course some of these others who are questioning are manifestations of David's mind and this is David's attempt to break free.

Oh. I misread. Thought it was obvious the monster was Lenny hence the authoritative position.
Still interesting as to why sid realises something is wrong but David and Marilyn don't.

I think that Lenny was a real person until she was accidentally killed by Syd.
The monster took on the Lenny persona from that point on.

Denny was the real person from the pre hospital memories. Lenny seems to have been a real person from the hospital, who was killed. The monster assumed her persona and replaced Denny with itself in his past memories, as a way of making David listen to him/Lenny and trust him/Lenny.

Read somewhere that the monster could be the "Shadow King" from the X-men comics. Does everyone agree with that or is it something else?

Marilyn mentioned before that he was a telepath and became obsessed with creating his own world in the astral plane, so much so that he eventually stayed there, almost forgetting about his previous life (I believe they froze him to keep his body from dying until they could find a way to make him remember the real world and come back).
We don't see a world he's created in the astral plane, just a block of ice he lives in that keeps him safe from the critter in David's head.
My guess is that the entity freeloading in David has taken over the astral plane (or at least the part where Marilyn's husband was world-building). Having just met David, he hasn't forgotten he exists and thus wants to help him (maybe the diving suit is an extension of the protection provided by the ice cube?)
If this is true then Marilyn was given a glimpse into what was happening in reality (and that the events going on in the "hospital" are happening in the split second between the guns being fired and the bullets hitting David) by her husband. I think the eyes opening at the end signified the creature becoming aware of ice-guy's interference.
Good, fun episode but I'm ready for them to be back to reality.
