best advice on how to pay off a large hospital bill.

A family member had an ER visit in between jobs, and was without insurance during that time frame. The bill was huge!I've read that the patient will not receive wage garnishments, and can set up a payment plan at about 10$ a month if a settlement is not reached. Does anyone have any advice or experience in this issue?

If no insurance and between jobs, may qualify for hospital charity for all or a portion of the bill. Have him call and talk to a financial counselor to see what options the hospital has.

They will settle with you for 50% and a monthly payment.

wait until it goes to collections and settle for less than you owe

Set up an auto pay for about $5-$10 a month. Give it a few months then try to negotiate. They'll settle.
ETA: not legal advice. Just something I did. I had insurance but still had a good bit outstanding and negotiated the balance down.

Can you get them to do something if you do have insurance and are still responsible for the 20%?

Depends on the size of the hospital bill but if it is too massive bankruptcy is not a bad option in some instances.

I looked at some bills I had a few years ago. They listed their charge and what the insurance company paid and it was about 15%. So, if you're charged 20k talk with them and get it down to 4-5k.

Sending the patient a gross charges bill is usually a scare tactic. If the payer happens to be self-pay, then anything is better than the $0 they stand to collect for writing it off as charity.

Sue them for malpractice. Settle out of court for legal fees and your medical expenses. Plenty of starving lawyers out there willing to help.
