i guess it depends on what parts of each city you are comparing.
i don't find the areas of New Orleans i frequent to be overly filthy.
But Manhattan might have to be the cleanest big city in the U.S. imho.

Rome was the same way. Took the Metro from the airport to the center of the city and thought i had gone to the wrong place once i made it out onto the streets. Trash everywhere, it's like they don't use trash cans.
The Vatican was amazing to see and beautiful, but anywhere outside of the Vatican walls were disgusting.

ETA: those listing US cities have never been to the Mediterranean. However, the nastiest US "city" I've ever seen is Holly Beach, LA.

In the US, it has to be New Orleans. I did not find NY to be all that filthy though like any big city it has undesirable parts. Haven't been to Detroit, Akron or Cleveland.
All that said, New Orleans is basically a sewer.
Internationally, I found that Seoul, while very interesting, was filthy. People in the US who complain about filth and pollution really have no idea just how clean we really are.
