This.

Also I'd put Mulholland Drive up there as well.

meh, emotional manipulation.

the "scream" part is retarded as it serves no purpose other than to shock you. it is out of place and really pissed me off.

There is no way I could ever watch Dear Zachary. The level of hate and anger would consume me i think.

Not sure if this would qualify because it's more depressing than disturbing I guess: Before the Devil Knows You're Dead

Final Destination (and pretty much all the sequels)

...these movies aren't traditional horror films but have very disturbing plots and themes. I watched one of the more recent sequels on cable and disturbing is a great word for the films. The whole idea of having visions of horrific, freak accidents before they happen was disturbing. The movie weirded me out almost to the point I didn't like it.

"When a man loves a woman" was totally disturbing... disturbing in that my "take my wife to a chick flick to set the mood" totally back fired. Its was described as a romantic drama... really nothing romantic and quite depressing.

Antichrist.. Probably hands down the most disturbing movie I've ever seen

edit: not horror or torture types, then I would have to agree with Donnie Darko as my most disturbing
