The main rotor hub is still attached to the mast. The nut is still retaining the hub in place.

Another site said you can see the transmission still attached to the shaft. I assume that is a huge structural failure of the transmission itself or the attachment to the frame.

And this is exactly why I won't fly in a helicopter that isn't owned and maintained by the military or the government. The amount of maintenance that goes into a helo, I only trust to an organization with unlimited funds and time. The second someone tries to pinch a penny or cut a half a man hour you can count on something bad happening.
In my prior life, we were about to take off on a mission and the pilot looked back at me (he was a buddy) smiled and said over the intercom: "Don't worry boys, there are only 11 thousand parts on this Blackhawk and only ten thousand and fifty of them are critical to us staying in the air." frick him, I am still mad at him for that comment.
