I miss the show seconds from disaster. It was all about stuff like that.

I listened to this audio book on my commute prior to pandemic. It's kinda overly detailed/boring but will educate you on TONS of shite you probably didn't know about the entire nuke program during the Cold War. Highly recommend, albeit on audio book at 1.2x's speed.

There is a pretty good PBS documentary on this incident (based on the book) called Command and Control.

I watch a documentary about this.

It was caused by 5 guys who wanted to get off of work on time on a Friday afternoon.
