No. This was never explicitly stated to my knowledge. Just that the swords were from the vanquished.

Correct.
Aegon the Conqueror had the Throne made from the swords surrendered by his enemies. It is supposed to have taken a thousand blades to make, heated in the breath of Balerion the Black Dread. The hammering had taken fifty-nine days.

Have you read the most recent novella from the Dangerous Women anthology? There's a part in there that may strike you as interesting regarding what Mirri described.

I have read TP&TQ. I know Rhaenyra had a scaly baby just like Dany. But is it mere coincidence or did Mirri not cause the stillbirth? Because Rhaenyra's baby was still born as well.

Do you think D&D did a little foreshadowing on their own?
I posted my thoughts on it a while back, although I know a particular lady on here who doesn't like the idea.
I think that it's almost all about Jon. She sees Snow on the iron throne, then she sees the blue rose at the wall, then she sees her husband and child at the wall. My theory (which matches my theory on the books) is that R+L=J, Jon is revealed as the rightful king and he and Dany end up together.

It seems to me like that's where we are headed. But then again I figured Ned Stark was the hero in this story when I was reading AGOT. Nope. I was wrong. Then I thought ok his son Robb is a badass fighter and he will avenge his fathers death. Nope. Wrong again.
It is called A Song of ICE and FIRE. Jon is Ice. Dany is FIRE. I would hate that myself because I think Jon deserves so much better and I don't really wanna see him on the Throne at the end. Possibly our only saving grace is this vision Dany has in the House of the Undying--

What do you think this Dany vision means then if we are to believe her baby was a half dragon deformity?

I think it's possible when a Targ has a baby in extreme anger, despair, or grief that it could have an effect on the birth. Mirri may have had a hand in it, I'm not sure. But I found it very interesting that they had similar babies that didn't survive.
As far as what Dany saw, in the House of the Undying she saw the past, present, and future. I think she simply was seeing what might have been. And seeing that the Warlocks wanted to keep her there, that'd be a pretty good way to do so.

So to me that means that the Song of Ice and Fire is at least not about R & L. I think its really about Jon Snow being the result of R & L but I can see where it may lead people to believe it means Jon Snow(Ice) ends up with Dany(Fire)

Pretty sure that vision is of Rhaegar and Elia, the baby he is referring to is Aegon. He says there must be a third head and that is the beginning of meeting with Lyanna to have Jon.

I'm almost positive it was Rhaegar and Elia with baby Aegon in that vision. If R+L = J, Lyanna would be near death after the birth...or on the express to death, anyway.
The many aspects represented by the title ASoIaF are what makes it so well-chosen. It could be about Jon Snow, the Targs and the Starks, the Others and the dragons, the sword Ice and it's eventual re-forging ( a very Tolkienesque touch if so) etc...
I think the title reflects the forging itself...people, civilization and the trials and betrayals, relationships and love...mostly that it takes these two destructive forces used properly to create steel, a substance stronger than the combined effects of both.
Goes back to Howard's Conan and the whole "mystery of steel"- we know GRRM a big fan - the truth is steel isn't power, flesh is power, but it takes steel to control it.
Something like that, anyhow.
