I'm not sure what you mean here. You aren't crossing the international date line. There will be some jet lag but you can soldier through it.

When we went, we landed in Rome around 9 am, dropped our stuff off at the apartment we rented, ate lunch, and headed to the Vatican. My sister in law had taken a Xanax on the flight...we got a great video of her passed out on a bench outside of the Vatican doing the head bobbing in her sleep

Naples is a shite hole. I was there last week. My recommendation is get in a car and drive straight to the Amalfi Coast.

I would rent a driver or get on the ferry. The road to the Amalfi Coast is not for the faint of heart.

Just that coastal road to the Amalfi Coast. You can turn the car in Naples (they drive freaking crazy in that city) and then ferry to your chosen town.
Tuscany is easy but I'd avoid driving through a city if possible. Rome is not a place I'd want to drive and at least some cities have times where you can drive on certain roads and times when you cannot. It's not easy for an American driver to pick up when it's clear to drive and when it isn't.

I contacted transferelax. We’ll see what they quote. Thanks for the rec! We may rent a car once up in Florence but I don’t plan to drive anywhere besides the countryside up near there. Like I said I don’t want to stress and want someone waiting on me to bring me directly to our apt in vietri perhaps handle luggage. I am not at my strongest since I had emergency surgery and it’ll be just me, my mom, and my teen daughter. After landing in NAP we aim to use vietri sul mare as a base for 4 nights exploring the coast, pompei, the Herculaneum etc. I have no desire to mess around in Naples from what I’ve learned here. Then we’ll head north to Rome for 3 nights via the fast train. We are staying right south of Vatican. Easily walkable area. Then we train to Florence to use as a base for 6-7 nights. In Florence we are staying right near ponte vecchio. All restaurant and experience recommendations are welcome for this itinerary!! Thanks to those who are helping to make this experience so fun for us.

We rented a car in Florence to drive around while in Tuscany for 4-5 days. When returning the car in Florence I apparently drove in a lane that was for workers/commuters only or something, and I got a ticket through Enterprise a few weeks after getting home. I paid Enterprise since they charged my CC they had on file, but I still get collection letters from the City of Florence 4 years later for around $250. I'm just going to bank on most likely never driving in Italy again and keep ignoring the collection letters

You asked for Florence restaurant recs. Checking my notes from 2018, we spent 3 nights in Florence and ate at these three places:
Ristorante Saint Ambrogio - Had a very good dinner there Easter Sunday night.
Ristorante Natalino - Another very good upscale gourmet bistro type of place.
Cacio Vino Trallalla - Tiny hole in the wall wine bar that sevves delicious food. It was right around the corner from our hotel.
Over two trips I have two weeks of experience driving rental cars in Tuscany. It is pretty easy for the most part. Be careful in remote locations after dark because a lot of the roads are narrow, winding and unlit. We encountered a few dirt roads. But it will be fine, especially if you have a good GPS.

I’m pretty sure I’m about to get a hefty ticket for driving through the old city in Sienna. The guy on the street looked like I was nuts when I asked if I could park where I was parked. He was like, you aren’t even supposed to be in this part of the city. Oh well.
Also, drove the wrong way down a one way in front of a police station today. It was in a little backwater where some family is from. The cop out front walking in from his car just waved back at me as I did a six point u-turn.

We did the same thing. We drove inside the old city walls of Siena where cars were restricted. We realized that we were not supposed to be there but we were having trouble finding our way out. We were about a block and a half from driving on Piazza del Campo, which really would have been trouble, when we took a sharp right turn and managed to zigzag our way out. We were expecting a ticket but never got one.

The driving in and around Sorrento was something else. Drove to Positano in the morning and back that night after dinner. Yeesh.

It would be tough for you to dispute but tougher for them to collect. I have spent two weeks over two trips driving around Tuscany but didn’t try it on the Amalfi Coast.

I just spent three nights in Varenna. It’s beautiful and well worth a day trip from Milan in my opinion. We took a private boat tour that I would definitely recommend if interested.

Lake Como is great. I'd stay there 10 out of 10 times if Milan was the other option.
