re: Was Hillary picking Tim Kaine the worst decision she made in the entire campaign?

this was the most fatal decision she made.
Keep in mind who you're talking about - she didn't have much choice. Her ego wouldn't let her pick someone who was more dynamic, popular and charismatic. There was no one acceptable to the Dem establishment to balance her out.
Picking someone who could actually "help" with the campaign ran the risk that person would overshadow her.
So, literally, Kaine was about the best choice she could make - unoffensive, bland, a pick that couldn't be considered a blunder or misstep.
"Kaine and Unable" - I still laugh about that.

No. Her picking of Kaine didn't help her but any hindrance was negligible enough to be non-existent.
The pick that I think hurt her the most was picking the disgraced Debbie Wasserman-Shultz to be an "honorary chair" on her campaign within hours of her having to step down in disgrace from the DNC chair. This pick was a massive middle finger to Bernie supporters in general but especially those that believed the DNC leadership was working against their own rules to stack the deck against one candidate (Bernie) and for another (Hillary). Many of those voters either stayed home or voted against Hillary strictly out of spite. Had she not brought DWS in like that she may well have been able to bring those Bernie voters under the Clinton tent.
The question after that is would that have been enough to change the outcomes in places she ignored (Michigan and Wisconsin)? She may well have won close races like Pennsylvania and Florida, but we'll never really know.

If not for the deal with Kaine Hillary would not have been the nominee.

I disagree completely. The leadership in the DNC was dead set on not allowing another Jimmy Carter to happen. While many look at it as the DNC leadership going above and beyond to make sure Hillary was the nominee, it was just as much to also keep Bernie out of that position.

Not sure America was ready for a third term for uncle Joe. Bernie would have been a disaster.
Booker may have helped. Warren...???
Kaine was average and boring. He didn't help or hurt IMO
