Small arms fire really wasn't as big of an issue during Desert Storm. Scud missiles and land mines were our biggest threat. You could also say that the Patriots knocking SCUDs out of the sky only to fall on or around us was.
Vehicle accidents, especially along Tap-line road (MSR Dodge) were the number one cause of death and injury among US Troops.
We only wore Flak Jackets when clearing bunkers and mine fields.
During OIF we wore our issued IBA and placed our flack jackets and sandbags on the floorboards and or strapped to the doors of our non-up armored vehicles. We used hillbilly armor until the real up-armor became available to us.
MRAPs were just starting to cycle into country as we left in '05. By the time we got to Afghanistan in 2012, MRAPs had totally replaced Hummers as the primary troop transport for off FOB missions and Hummers were not even allowed to leave the wire.
TL/DR US Troops are some pretty resilient and innovative people. You could give us the latest and greatest and we will still find a way to improve or expand upon our issued equipment.

Ok so maybe it's my memory, then. It slips a lot.
I distinctly remember people going around, trying to gather money to buy vests and other gear shipped overseas. I probly got the engagement mixed up.
