I saw a report that says it was a "friendly fire" incident.

Yall keep praying for my friends. Young operators, especially foreign, on defense systems are frequently the most dangerous thing we deal with.

I'm not really sure how Pahlavi could form or lead a transitional government;
simply put, Trump can not start dealing with him formally now and send him over, or he would be acting as a kingmaker (sending the deposed Shah's son over to reclaim control of the country). Pahlavi says he only wants to help the transistion, but you can't know what he would do once in power.
I think he'd have to get his arse over there somehow by himself, and raise popular support, and then we could choose to work with him. But at that point, would it still be considered "transitional"? We need somebody to surrender, or someone to take power and say "come in, we have it under control now".
And I don't think we can just bomb them into the Stone Age, and then quit and leave without somebody in charge.

There is a long way to go before the current government abdicates, if they ever do. Even though they are greatly outnumbered, the IRGC is too well armed, and the general public are not.
The best-case scenario, in my opinion, is for the Iranian military to turn against the IRGC / government and perform a coup.
