Check out Medishare. Not a recommendation per se. I literally know nothing of the particulars. But I understand there are many satisfied customers

Why don't insurers have an app where you can key in the NPI, ICD-10 codes, and CPT codes and it can tell you your out of pocket?

United healthcare has a Surest plan that does this. There is an app and you can shop the lowest price. For example you need an XR you can see what imaging facility has the lowest contracted rate. There are a few annoyances about it (surgeon doesn’t operate at the cheapest facility so you’re paying more for somewhere else, and getting the provider to send the order to another place etc. ).

Well every policy is potentially different in terms of what it allows and pays. BC/BS has something like 55,000 different policies in the US.

They do have websites where you can look up your specific policy but it doesn't usually tell you much, and they always carry a disclaimer that basically says, "We can decide to not pay for something even if your policy says we pay for it." That's a paraphrase, but that's what it means.

3rd party pay is the worst.

The healthcare industrial complex basically normalized dynamic pricing.

I don’t trust any of it. I have several doctors in my family but I still avoid medical professionals. It all seems like a big scam.

I do think you have integrity though, NC, and I would trust your professional opinion
