Look...in all honesty, I really do feel for anyone that's afflicted with this issue. Same as I would any person with a serious mental or physical problem. I wish that there was some pill or medication that this poor guy could take which would completely alter his DNA and he'd TRULY be able to become a woman if his brain has been wired in such a way that he's always felt that way. As a man that's VERY much a guy, I can't begin to imagine what it would be like to feel as though you're genuinely in the wrong body.
But until then, this is still a man that think he's a woman. And that's true whether he keeps his genitalia or decides to have it removed and have a surgically created "vagina" in it's place. Some people will decide to be "polite" and act as though he's a woman, and if they want to do that that's absolutely their right.
Personally, I'd refuse to do that. Not out of any desire to make this man's life more difficult or to be rude, but because I actually care about the language and am concerned by the obvious attempt to make people say and do things they KNOW are false but do them anyway.

That's exactly like being a child of or a submissive partner of a psychopath, one has to agree with their obvious blatant lies or there will be hell to pay, if they can inflict it.
