Approval rating is not the same as casting a vote. Up or down are the only choices.
When the choice is between Trump and another candidate, the percentage that will vote Trump will be much lower. How much lower will depend on the candidate.

That wouldn’t be statistically impossible with the US demographics and party size.
People in the south think this because at the state level in places like Mississippi, Alabama, and Louisiana, that number is pretty close to the truth.
