Presbyterian is the style of church governance—elder led with a hierarchical presbytery over the churches.
Reformed is the theology—Calvinistic in its soteriology.
There are a lot more distinctions that can be made like Dutch reformed vs Scottish reformed, Heidelberg vs Westminster, etc., but these are pretty minor.
For example, you can have a reformed baptist church. Reformed Baptist has the same soteriology as Presbyterian but does not hold to infant baptism. They also are not under a presbytery in their church governance.
