Anyone who is extremely sick can have blood clotting issues. It’s called DIC (disseminated intravascular coagulation) which can happen from infection, trauma, or cancer. He either has that because he is so severely sick from the virus or it’s a complication of ECMO or possibly pressor medications he is on.
Either way, the entire first page is trying to discredit his illness or joke about his wife. I think the denial And attacks have been ratcheted up significantly because he appears to be a pretty healthy 41 year old on deaths door.
Pretty sick if you ask me.

The medication is keeping him alive. Without the medication and the huge machine artificially putting oxygen back in his blood he would be dead. The medication is necessary to keep his blood pressure high enough so that he doesn’t die. It’s sacrificing his extremities to keep blood going to his brain and vital organs.
