Are you an employee or contractor? Those are two different classifications. I assume contractor, if so, you need an attorney.

I basically rent her office hourly. Instead of a flat rate, she gets 30% of what I make. She takes the co-payments and handles billing then cuts me a check. I previously told her if delayed payment continues, I was going to hold on to the co-payments until I received reimbursement. She stated that she did not take kindly to "threats" and would consider that stealing. It's not stealing when she can just deduct the co-payment I held on to from the remaining insurance money.

What does your contract say? Breach of contract claim or contact the Lousiana Workforce Commission.

lawyer up. sounds like the people who owe you are about to go bankrupt.
I briefly worked at a Diebold contractor and busted arse and was known by all the Diebold people. My boss took the money and became a pill junkie, (not good when you are doing bank security). He got released from contract and was over a month behind on pay and the Diebold employees and managers I had got to know all acted like they didn't know me and none offered to help out.
The main company just said they fulfilled their contract and that it was up to my boss to fulfill his

Well here's your problem... a flat fee is way better than someone else handling your money and expecting them to cut you a check back IMO

The contract stated she has up to 7 days to submit claims. I went to the LA Workforce Commission page. Which of these do I contact?

I agree. I will be handling billing on my own at the new place I am renting from. I only have to pay $18/hour. Before, 30% came out to $30/hour.

If you contact the LA workforce commission it's going to really piss off the persons you work with. So will getting a lawyer involved. It will end the relationship. I'm a lawyer and work in this area and see this all the time. You need to assess how much you are making and how much you would lose if the relationship is severed and work from there. This is a tough situation.

When I complained the first time she made up a revised contract that said she would have up to 14 days (instead of 7 as originally agreed) to submit claims. Again, they take 5 minutes per claim. I refused to sign it. She had excuse after excuse saying it's been very hectic lately (she moved, she was sick, vacation, she works until 9, etc). That is not my problem and I should not be punished for her lack of organization.
The relationship has already ended. Next Thursday is my last day.

Ah, she's one of those peeps who contracts out workers to job fields.
Had a buddy who's wife did that, she contracted Doctors to Hospitals. She got $25 n hour for everyone that worked. She raked in a shite load of cash.
Her stock options alone were worth over 2+mil when she retired/quit. He married well. I admire it.

The relationship is over, they have parted ways and the other party has already got their lawyer to send him a nasty gram. The lady still owes him money, time for a certified demand letter

First read and understand your contract, it is all that matters, any other verbal agreements y'all had are null n void. Make damn sure she is in violation of the contract and that you are not. Talk to a lawyer and get opinion and they will draft a letter, if you choose you can google a demand letter and if you feel confident enough make your own, but be careful about how you word it and what you demand, it can come back and bite you in the arse, send it certified mail, return receipt

Ahh, I didn't read everything my apologies. Lawyer up and get the lawyer to send a Dunning letter. If no response, file suit under breach of contract or on an open account. Sorry this shite is happening.

Go talk to whoever you report to or whatever.. first thing Monday morning. Be cool about it, but tell them "look, I haven't been paid since June 23. I can not get answers and I need to be paid so I am not leaving until I can find out exactly when I will get paid. Give me my money so we can just be done with each other"
