Safety rep, PIC, Field Foreman, Consultant & Supervisor all ignored the weather conditions. Some how the guy was able to grab his SRL while falling, catch his balance on a beam & pull himself up. Had his SRL failed & would have free fallen, he would have crashed on the plus 10 then been washed away by the ground swells without a vessel nearby.

They have an anonymous tip line for shite like this. I'm all for manning up and getting the job done at some points but this is absurd. You don't start to effect change unless you start somewhere. I know the culture is shite out there (from experience) but if this story is true it should've been reported to someone who can actually do something ASAP. (Sorry for the preaching)

I helicoptered in, took the field bird to the satellites, got grounded from the weather and had to crew boat back

You haven't lived until you board a helicopter that has to hover because the wind wants to push it off the pad. I'm not big, I had to get get someone to hold me so the wind wouldn't blow my arse overboard lol. Then the helicopter doesn't take off straight up, it's like it falls over and then takes off. Your heart skips a few beats with the turbulence. I loved helicopters. Get there fast and get home fast.

So maybe I am not looking at it right, but it does not appear that the guy in the picture being pulled on to the Coast Guard ship is wearing a PFD? In a situation like that would a PFD be mandatory ?, would most be wearing one?

PFD would not be mandatory on your body during transit unless requested by capt. The fact that he has no PFD on hints to me that the boat capsized very quickly, although I'm just speculating.

We were too heavy on the little chopper one time and we did the whole fall off the side for airspeed thing. All I heard on the headphones was "Don't Panic" as the gulf became too close to my for comfort.

I'm fully aware of BSEE's hotline. So are other workers. Guys are just fearful of the repercussions of "rocking the boat". Personally, I refused to get on the scaffolding but other guys did it. And yes, this is indeed a true story unfortunately.

Looks to me like, sadly, they didn’t have time to get them
I would imagine that, most of these guys were hanging out in the galley area.

How about flying in heavy fog on a 76 & neither pilot can see the helipad of the the platform? That definitely made my a-hole pucker up.

I don't want to get too in the weeds with this because I do respect the men and women, like yourself, who are still out there to make a living. I've unfortunately been a part of too many fatal accident investigations that would've never happened had someone just spoke up to the right people. Its easy to say, I know, but we have to start somewhere.

What is your best guess or explanation what happened to the crane that was around the leg?

I know & honestly respect your knowledge & encouraging posts. I also know I should have said something because the last thing I want on my conscience is a fellow worker dying when I could have done something about it. No man or woman should ever be put in a position where their job & income is threatened for doing what's right but this is something that occurs on the shelf on a regular basis.

Thank God for radar right?

I hated fog because some pilots refused to fly in it.

Im going to guess that the crane is not bolted to the leg. It slides on so they can easily remove it for transport.

What exactly happened? I must have missed the discussion of that this thread.

“ We were too heavy on the little chopper”

Between everyone “losing weight” to catch last ride in and the “borrowed” tools in the bags we were always a little heavier than what was written up.:)

It's a symptom of the good ole boy network of the oilfield.

I will definitely be waiting on the root causes from yesterday's weather related events. Hell, some of it will most probably become training material. I have a few dumb stories about decisions on the shelf but I'm thankful that they were never serious. Deepwater was a much more pleasant experience (including the smoking rooms on the out of country floatel)

I had to do that rope swing bullshite once to get to a toadstool with a broke dick aircraft. Seas weren't that bad but I will never ever do that again. Visions of being crushed between the boat stern and the landing were very vivid. And it was winter time so that made it even worse. Id ride the lift ring anytime.

An air log helicopter blew off the pad during lunch one day because the pilot didn't strap it down while he went eat.
