Three dead, two injured following scaffolding accident at Port Arthur LNG

At approximately 2 a.m. on Tuesday, April 29, an incident occurred at the Port Arthur LNG construction site involving employees of engineering, procurement and construction contractor, Bechtel. Three fatalities have been reported and two others who were injured have been released from the hospital. Our condolences go out to the families of those who lost their lives as well as those who are injured. Bechtel has suspended work on the site while the incident is investigated. Bechtel is working with the applicable regulatory agencies and local authorities to investigate the situation and will provide further updates as they become available.

9 times out of 10…..it’s scaffolding crew related.

OSHA fixing to eat someone’s arse.

Might even look at criminal liability for something this bad.

And if that scaffolding crew has some undocumented “visitors” on it buckle up.

Scaffolding seems to be done at safety levels that wouldn’t be acceptable elsewhere. It’s like everyone agrees to turn a blind eye to it when scaffolders are building scaffold

Looking at that picture it seems to be more of a shoring type scaffolding than a regular access type scaffolding.

Whole different animal. Probably was whoever was building that tank more so than a scaffold company.

Most of the risk comes from the fact that nothing is fixed in place....that's what the job is doing. fixing things in place. There is also inadequate places to tie off....again, installing places to tie off is part of the job.

I had one of the best scaffolding GFs I ever met in a crew 7 years ago. His main focus in life was determining if a scaffold was even necessary....and often it wasn't, there were far safer ways to get to the proper elevation. The problem was the work planners and field engineers weren't having to build the scaffold so what the frick did they care? They had never even built a rolling scaffold and their sole experience with scaffolds were with finished scaffolds which are incredibly secure and safe...its getting them there that is dangerous. This was Bechtel project also...that old man was one of the best trade GFs I have ever worked with....

Also happened at 2 AM. You can rest assured that there are a couple of things going on at 2 AM...one is all the truly risky shite is being taken care of because there are fewer people around to see and tell. The other is there was probably one, if that many, safety people on site and they were most likely inexperienced since they were working nights.....

There is an advantage to having back shifts on sites which are heavily driven by processes....its easier to get away with shite when 80% of the job is at home asleep.....anyone who has ever managed a night shift knows that this is often a suggestion by their day shift counterparts.....

A buddy of mine did scaffolding and I wouldn't trust him to build a lego set.

That sucks.

The LA Capitol is about to have scaffolding going all the way from the steps in the front to pretty much the base of the lantern at the top. The engineer designing it knows his shite but it is gonna look so fricking sketchy, especially when guys are pulling stones off of the 30th floor.

Just saw that a crane may have dropped a load on the scaffold. After seeing the pictures that makes more sense.

Wow that would be extremely unfortunate. Operator error or the crane had an issue?
