Elderly relative went in for a regular blood test, got a call back saying she has anemia and needs to be in the hospital. Scheduled for a transfusion. No other symptoms, feels fine. If it makes a difference, diagnosed with epilepsy about 5 months ago, put on Keppra which controlled the seizures. Only other medications are an antidepressant, statin, and blood pressure medication.
Any thoughts?

Possibly an internal bleed. Your systems decline with age. Maybe she just isn't producing enough red cells. Possibly bone marrow has something else growing in it like a chronic leukemia. Need WBC population numbers and a bone marrow aspiration to diagnose.

This was sudden onset, FWIW. She has been getting regular blood tests to monitor the Keppra level and everything was fine up to now. I notice anemia is a side effect of it, but pretty rare.

How severe is the anemia? Do you have Hemoglobin and Hematocrit numbers from a couple of months ago and recent values?

Always have to exclude GI tract malignancy in these scenarios.

Based on the information you provided, she likely has anemia with epilepsy and a touch of depression. This usually leads to an increase in blood pressure so she is likely at a much higher risk of heart disease.

Saw her internist, he didn't seem super concerned. Will run all the usual tests, but his first thought is diet related. She has had a lot of dental work and hasn't felt like eating much lately.
