I do quite a bit of OSHA work. Over the years, these terrible deaths come to mind:
Man is in a hog skinning machine that was not properly tagged and locked out. Someone turns it on, man is skinned alive.
18 year old worker driving a large earth scraper at a landfill runs over a co-worker, literally "scraping" him.
Helicopter mfg. installs the wrong coupling into the back up hydraulic system for a helicopter. During a test run at low altitude, the main hydraulics go out. The backup switch should be instantaneous, but it is delayed due to the incorrect coupling. Pilot loses control and helicopter crashes, killing everyone.
