The pledge is when it's standard to put your hand over your heart. The anthem: stand and remove your hat. For baseball, we would put our glove and hats at our feet and our hands behind our backs. I still do it that way

Hand over heart is not required to show respect. It is something people started doing several years ago and it caught on....sorta like flat billed caps. Its trendy but not part of the Nation Anthem official protocol.
