There are two aspects to putting. The line and the speed, with speed being the most difficult.
An old guy taught me about speed. Once you pick a spot between the ball and the hole, get over the putt and aim. Then...forget everything. You may have thought beforehand about uphill/downhill, fast/slow and grain...it's still there subconsciously, but forget it.
Then make the putt. Watch the putter hit the ball, back off if you see the putter not perpendicular (larger grip has helped me with this and I am a straight back/straight through, not swinging door).
It's the "feel" game and mechanics will only make you worse. I use the same thought process when I pitch/chip. I think about lie...deep/tight, obstructions, am I short sided or can/will it run. The thoughts are there, forget them and hit the shot.

What helps me as far as alignment is concerned is that once I have the ideal line, I pick a spot on the green 3-6ft from my ball to aim at. There is always some kind of blemish in the green (brown spot, small piece of a leaf, ect..) that will fall within 3-6ft from your ball on your selected line. Now just hit that spot with the correct speed and as long as you had a decent read you should be good. Doing this helps me out tremendously when it comes to alignment. Much easier than just aiming 3ft to right of a hole 20+ ft away.
Of course your line means nothing if the speed isn't correct.

Your putter head has got to be square when it contacts ball.
Period.

A good drill to ensure square contact is to place two balls side by side just touching each other. Set them up about 2 feet from the hole and putt. If both go in the hole you took a good stroke.

If I focus on a spot a few feet in front of the ball, I wind up focusing too much on that and screw up the distance.

I use the feel method a few people mentioned. I like to pretend I would be rolling the ball with my hand into the cup and replicate that feel with the putter. I had taken much advice, but one instructor gave me this advice in a session and it worked well for me. Longer the putt the better this works for speed.
