Co2 is used to insufflate laparoscopic cases in the OR.
This could have been staffer error....or a co2 tank could have been accidently or intentionally cracked....
Don't know hoothat could have done much damage in a large ER though

"Casualty" doesn't mean fatality, just means they were injured or affected by the event.
Someone who loses a leg is considered a casualty of war even if they don't die from the injuries.
