Two things that I know the Feds have been working.
1. Prescription wholesalers being tasked with ratting on the pharmacies with high opioids fulfillment.
2. Pharmacist being tasked with ratting on the Dr's who write the most scripts which would lead the Feds to the pill mills.
I know a couple states, like West Virgina, have included prescription wholesalers and pharmacist in lawsuits. The lawsuit claims the wholesaler and pharmacy have a responsibility to report Dr's who write high levels of opiate scripts.
Of course, neither of these address the problem of cutting a bunch of addicts off. These just attack the supply side with not enough care for the addicts. The past couple years have proven, cut off the prescription supply and addicts will move to the illegal side.
I was in a pill mill a few weeks ago. It was shocking to see. I went in for strep throat In a walk in clinic. I saw at least 5 to 8 addict looking people come in and all paid cash and spent about 5 to 10 minutes with the dr and walked out with script papers. I was only there a little over an hour.

This is incorrect. I had an epidural injection in my neck the Friday before labor day. The injection brought out a case of the shingles in my neck and shoulder. I went to a urgent care facility on labor day to see what was going on and they prescribed me Hydrocodone as well as an anti viral.

I was prescribed Oxy when I had my first kidney stone, I think I still have most of the script. I absolutely hated those things, especially when I started trying to shite concrete bricks. I sure wish someone would have told me about the side effects!

In the ER the problems are Press Ganey scores and salary is tied to patient satisfaction (The doctor was rude is patient speak for didn't get my Norco script)
and JCAH with pain being the "5th vital sign" (which is bullshite)
There is a Prescription Monitoring program in Louisiana and Alabama just need the patient name and birth date.

The fact that this is the case in healthcare now is just insane and makes no sense. You know it is, your coworkers know this....anyone with common sense knows this.

Agreed. I certainly think true pain needs to be addressed...but we are doing a horrible job in healthcare of educating people on the realities of pain. Pain control isn't about being 100% without pain. Depending on what is going on with the patient...that simply may not be possible.

A friend of mine is a pain management physician who specializes in procedures to help with pain management, rather than medications. A push toward that method would help.

100% due to privacy concerns and HIPAA. There are regional organizations that many healthcare systems take part in, but nothing as far as a national database is concerned. Also, healthcare systems that share the same EMR can pretty easily share info, but you have to know which hospital to request info from.
