I hear you. All I'm saying is that I haven't.
Yeah a brand new intersection probably would cost 100k but in this case, I think all they'd need to do is re-stripe the road, replace one signal head and probably do a little rewiring in the cabinet.
I do like redstick's roundabout solution though.

I've usually seen the middle lane with a left arrow light and warning that left turns yield except on arrow in that type of set up.

I don't know why everybody freaks out about roundabouts. We have a shitload of them out here and in low-medium volume situations, they're way more efficient than a signal. (And this is coming from a guy whose company makes signals.)

Any chance the guy in the middle lane needed to make an immediate right hand turn after going through the intersection?
We have a similar intersection in Houston, but there is a very popular body shop on the corner. So there are usually a few cars in the "middle lane," waiting on the arrow light because they are turning into the body shop almost immediately after the intersection.
It is aggravating as shite and causes quite a few wrecks. The light to go straight turns green first, so the people assume the turn signal turned green too, which it doesn't until later. Then they proceed to turn in front of oncoming traffic.
