The New Madrid earthquake had a lot to do with it. The course of the river cut off that part of Kentucky from the rest of the state.

Louisiana and Mississippi have a bunch of bends that look like that, just not nearly as large.

Minnesota has one of those anomalies too called the Northwest Angle.
I'm not sure how it came into being but you have to go through Canada to get to it via dry land.

I tried to drive to one of these near Tunica, a part of Arkansas on east side of Ms River in otherwise MS... it was fenced off, no access.
