Try Abby Singer's Bistro downtown. It's a nice spot with good food and feels casual, and you could catch an indie flick at the film center, if you're into that.

Most girls love Cheesecake factory, both food and desserts. If you really like her you'll be nervous no matter what. She knows that and she'll give you a pass on the first date so don't worry too much.
Brainstorm and write down about 20-30 light (not too personal or serious) questions to ask her for something to talk about. You don't have to use all of it. It's good to have it as a backup plan in your back pocket. It will also save you from having an awkward pause.

You don't go fancy. You go for a sit down "diamond in the rough" type place that isn't too gritty, but has some great food. That's what makes places like Pinetta's so popular as a first date restaurant.
As for activities, don't do a movie. A first date is all about getting to know one another, so do something that's fun and offers room for conversation. Most sporting events are usually out of the question, but baseball with it's more laid back feel and periodic breaks between the action is perfect for fostering conversation.
Playing sports can also be fun. A round of golf may be too long and arduous (unless you are both hard core golf enthusiasts), but a couple rounds of mini golf would likely be perfect. Go-Karts, laser tag, ect can all be fun. Letting your inner child out is all good on a first date and doesn't set the wrong expectations.
Don't do bowling right off the bat. Save that for a group date later on when introducing her to your friends, vice/versa, or mixing groups of friends.
Skating can be a fun date, but I don't think it works as well as a first date. If one of you aren't very good, it can be uncomfortable. Save that for a few dates down the line when the two of you have developed some trust for one another. Then, if one is terrible, it won't be as embarrassing or uncomfortable holding hands and/or helping each other up off the ice/rink.
Paintball is often too painful and competitive for first dates, but can be outstanding later into the relationship.

I am definitely in the no-restaurant for a first date camp. Especially if it's a girl you don't know that well.
Find out what she likes to do. Probably my most successful first date ever was a baseball game.
If you do choose a restaurant, I would recommend something like hibachi, where there's something going on, it's fun and there's not exclusive focus on each other.

I've done this before and it was great. My problem is some of these date things are "fun" but I don't know if I connect to her in conversation because everything is already orchestrated for you via the activity. A baseball game is slow enough to have a ton of time to talk about non-baseball things.

some of yall must have bowels of steel. Whenever I drink coffee, I need to be within 10 minutes of a restroom. There's no way I'd invite a first date for coffee, considering I know I'd have to poop shortly after and that's not a desirable thing on a first date. Plus, coffee breath.

My first date with my current gf was frogging with a few of my friends and her teammates. She from out of state way up north as are most of her teammates as they are all here to play sports and we went out to the family property. Was something none of then had done before and there was plenty of 'new' things to keep her interested. Was a complete success. All of them loved it (beside the plethora of misquitos) and one of her friends eneded up dating mine so now we have double dates if we get bored.
No high expectations, she fell in love with Louisiana, we were doing soemthing I enjoyed, plus we cooked up the frog legs the next day. Only reason i consider it a date is because we had one of the boats to ourselves so there was alot of alone time to talk. Plus she knows I would have gone whether she accepted the invitation or not.
