A family in my hometown had a run in with a log truck.
As they passed on a bridge the logs came free. Killed the mother and I think the sister. Brother survived (or maybe it was the sister).
Awful.

I saw an 18 wheeler take out a deer right in front of me in Nebraska on the same trip I almost died on the black ice in South Dakota. I started to get the feeling I wasn't making it back without a mishap

Blowouts are easy to handle, I've had two. All that happens is there's a sudden drag on one side, just hold the wheel straight and slowly steer for the side of the road. Easy. Too many people panic, overcorrect, and get into major trouble.
What does bother me is driving on a wet highway where all traffic is going at fairly high speed and there is nothing to separate opposing traffic besides a yellow line. All it takes is for someone to slip on a curve and you have a head on collision. I tend to stay in the rightmost lane.

Driving next to semis on a bridge or in a tunnel. When I was a kid, we were on the Causeway (single bridge, opposing lanes) and a truck driver apparently fell asleep at the wheel. He was barreling toward us and we had nowhere to go. All of a sudden, lightning struck, and the loud clap of thunder woke him at the last moment. He swerved and missed us by inches. Scared the crap out of all of us.

I tend to white knuckle over the Ms river bridge and the causeway. I have a fear of going into the water or being stuck on it somehow.

I have a hole in my Tahoe grill right now from an 18 wheeler tire blowing out in front of me. Shot rubber into my grill and tore off this small plastic skirt under my bumper, at the time I thought it was my actually bumper dragging so I pulled over. I had to pull the tire out of my grill and it dented my hood. shite freaked me out

Wheel falling off at speeds. I had it happen one time. Not much scarier than seeing your wheel pass you on the interstate while you just grind around trying to regain your control.
Stupid riverside towed me sideways the day before and I still blame them for almost killing me.

Spare tire falling out from the bottom of a truck or SUV.
I have seen it happen twice. The first time it hit my tailgate and went through the windshield of the care behind me. The second time it missed the Rv in the front then rolled across the median then missed the Rv on the drivers side but hit the trailer I was pulling. The Eighteen wheeler behind me jack knifed and caused a massive accident.
