Lots of predictable anger on this thread. Fact is, waiting for a provider sucks no matter what and everyone's time is valuable.
It is very hard to make a health care office run right on time. Very few offices purposely "stack" patients. On a day to day basis the office gets behind because of a myriad of reasons...patients no show or are late, they may have more complicated issues, emergencies may arise, etc.
You might think the best thing to do is to schedule fewer patients each day to make sure the office stays on time. If a provider does that then patients will bitch that they can't get an appointment quickly enough. If the provider tries to see more people each day to accommodate everyone then the patients will bitch about wait times. You try your best to find the best balance.
I agree that if you feel your provider is not doing a good job for you...find someone else!
Best thing to do? If at all possible always try to be your provider's very first appointment of the day...much less chance of a delay.

You occupied 1 hour of their time by not showing up.
Other practitioners that are obviously overbooked and always way behind need to understand that we may have 1 hour in our day, but if something comes up and we dont have 3 to waste at their office...we cant come in that day.

I know of a dentist that had 2 appointment times, 8:00, 1:00, booked 30 people or so for each slot. First come first serve.

Had to leave the office mid day last week because of a migraine.
Patient asked if she could charge me for cancelling at the last minute.
I took the $85 off her bill.

I would call them and politely ask them to remove it.
If they do not, then tell them that is the last payment they will be getting from you.
That being said, you should have canceled. I would certainly apologize for not doing that.
