I've got you by a few years and still use these terms when talking to clients even if they are younger than me. I was brought up like most in the South that these are terms of respect. From what I understand the people in the "North" don't like these terms.

If my children didn’t say ma’am or sir they were punished. I would have a long conversation with the principal about this teachers misconduct

First off, I will continue to believe there is an inverse relationship betweens one intelligence and their ability to be offended by petty things, and if the story is true, and something tells me there is more to it than that, but if true then the teacher is a moron.
To those saying it is the teacher right not to want to be called ma’am then their are three test I would ask if it would outrage you.
1. If the teacher preferred to be called a cuss word and have their students refer to him/her as a cuss word would it be acceptable.
2. If we were to look at this as harassment, from a legal term then would it qualify.
3. Is this teacher improving this kids future by making this request? At the end of the day the role of teachers is to prepare students for their future.

as learning to submit to authority figures outside the home.

What? My kids are not taught to submit to anyone, and to always think abstractly and objectively at all situations. I am preparing my kids to become leaders. Not submitters.
That doesn’t mean not listening to others advice, especially those with more experience, but I don’t teach them to submit
