Calling MD's/Med Students

My girlfriend (no pics) just failed the 1st step exam. I don't know much about it, but I know she basically has to be picked by a hospital to do her rotations/residency, I don't know the difference between them. How will this effect her being selected by a hospital? How does the selection process work? She wants to be go to a hospital in New Orleans or Baton Rouge, what are her chances of this?

It will limit the fields and residency programs that will interview her. She will need to apply more broadly to make sure that she matches somewhere.
The good news is that most people don't want to match in Louisiana. What field does she want to go into?

Plenty of people have matched into shitty Internal Medicine Programs after failing step one. Hope you like North Dakota.

She can still match in in New Orleans/Louisiana, but realistically her choice of fields has now dwindled dramatically to something like psych or family medicine.

She went to the first part of med school in the Caribbean, then the second part in Maine at their US location.

If she went to a Caribbean school and failed step 1, she should count her blessings if she matches anywhere.

There is one this year, but I guarantee you he didn't fail step one. There's 0 chance she will match in IM in New Orleans or Baton Rouge with a failing step 1. Lafayette is still a possibility since it is mostly IMGs.

The score on the step 1 exam is possibly the most important thing when applying to residency. She is probably going to struggle to get into a competitive residency since she failed her first attempt. She needs to pass it on the second attempt and hopefully pass with a decent score. The consequence now is that she will probably be doing primary care (internal medicine, obgyn, family medicine, psych), possibly in an undesirable location. Try to comfort her now because I'm sure she is devastated. She needs to absolutely kill it on her clinical rotations now. She should be the first to arrive, the last to leave, and do everything she can to impress her attending physicians and get glowing reviews. After step 1, your evals from your clinical rotations are probably most important, along with grades and step 2 score. She might want to apply to 150 residency programs in family medicine and internal medicine and hope for the best.

I'm really not sure, she said radiology at first, but now she said she wants to be an emergency room doctor. I don't know exactly what that entails but I'm assuming no specialty and less schooling.

She won't be matching into either of those. She needs to decide between family med and psychiatry and apply very broadly as well as do as well as possible on her clinical rotations and remaining step exams.

You can go through all of med school and not match for a residency??? What do you do then???

Yes. If she does not match into a residency, she cannot get a medical license in the US. She'll be an MD, but unable to practice.

Yeah, it's pretty obvious she is interested in the less schooling part.
