Depends on the doctor and what else has to be done. Simple implants cost less than implants with a lift.

Some women do it for legit reasons.
My wife got implants before we met because her boobs were uneven and 2 different sizes. She said one was a big B and the other was a C.

That's... normal.
The "legit" reason women do it is that they want attention/resources from those they consider more desirable than the person(s) they currently receive it from.
ETA: obvious exception for mastectomy patients
