I don't know how everything happened, but it only takes around 7-10 seconds to choke someone out by putting pressure on the carotid artery. The bouncer could have spent the first couple of seconds thinking someone was just messing with him - especially since it was completely unprovoked from his perspective.
It would still be pretty embarrassing, though. She's lucky he didn't just flop back on her and smash the back of her head into the ground.
