I have seen almost half of the movies on this list, and I didn't think any of them disturbed me enough to be on a top 10 list. Granted I haven't seen the top 4, but I have watched A Clockwork Orange, Hard Candy, Audition, Blue Velvet and Oldboy. Audition had a couple of gruesome scenes and the plot of Oldboy is certainly meant to disgust the audience, but I wasn't emotionally invested enough in the characters for either film.
However, I watched this film called Turtles Can Fly a few months ago, and it was very disturbing.
Spoilers below
There is no nudity or sexuality, very little cursing, and most of the violent acts are implied. However, the film was very hard to watch. It's about a group of Kurdish kids who live in a refugee camp in Northern Iraq, in the weeks preceding the first Gulf War. Many of the kids are missing limbs because of the high number of landmines in the area. In order to earn some money for themselves, the kids look for landmines, so they can sell them to a guy who works in the black market.
A young girl (maybe 11 or 12-years old) and her slightly older brother who has no arms move to the refugee camp, and one of the older boys has a crush on her. He doesn't understand why she is so detached from the world, and why she hates her little baby brother. The audience learns that the baby is not actually her brother, but her son. Although the rest of the kids in the movie never learn this, she was raped and impregnated by Iraqi soldiers in the aftermath of her parents' death. Several times in the film she attempts to either drown the baby or throw him off of a cliff.
If that's not enough, many of the child actors are maimed in real life, missing arms, legs, eyes, etc. Needless to say, it is very hard to watch and much more disturbing than some mindless, glorified S&M movie like Hostel.
Gummo is also disturbing because I genuinely don't believe that many of the minor supporting characters in the film are acting. They are just following Harmony's prompts and behaving like slight caricatures of themselves.

Hard Candy should be removed from that list. A Clockwork, while I love it, is faaaar more disturbing than the Juno chick castrating a lester. I wasn't disturbed in the least bit...a lil twisted, but hardly petrified.

One of my aunts actually got to go to the execution.

ETA: Oh, and the blood in the pictures was because he had a pretty high blood pressure.

What were you doing watching that, SFP?
The ending to that movie is on YOutube...and it appears as though the movie is centered around nipple sucking and knocking people in the face with a rock.

i have ichii, and it's one of my many DVDs i bought when i saw it on sale that i have yet to watch
but audition was not too bad. just that 1 scene with the guy in the bag. other than that it wasn't REALLY disturbing as far as horror movies go
