The winner is unequivocally Zelda from Pet Semetary. She still haunts my dreams. That movie remains the most haunting and depressing nightmare I’ve ever seen.
But as a younger child I’d add the wolf from Neverending Story or the Wheelers and Headless Witch from Return to Oz.

I've mentioned this before in other threads but the killer in "The Town That Dreaded Sundown" scared the living shite out of me. I watched this when cable TV had just come our and I was around 13 and had no business sneaking late at night to watch this. Because it's based on a true story (loosely) I thought the killer was going to get me.
To this day I have no desire to watch this 1976 film.

I watched the world premier on TV at my buddy's house as a kid, and Thriller frickED. ME. UP.

Damn near everything Dorothy encounters in Return to Oz is straight up nightmare fuel. The faces in the rocks used to freak me out too as did the idea of being turned into a decoration for the Nome King. Being introduced to electric shock therapy as a kid was pretty frightening too.

I remember watching the Terminator when I was 4, that was one of the first ones I remember seeing in life. Arnold/T800 skeleton was terrifying but awesome at the same time.
