Based on what friends who worked on the film have told me, it spans across several decades of his life so he's probably in a fat suit. Can't imagine him being the same build for the entire film because evidently they went all out to recreate each decade.
Plus the fullness of the face doesnt really match the bulk of the torso either. In fat suits, you often notice a contrast between face size and the body padding.

Well hes been on gear for years now. Expect a similar reduction in physique as the Rock one day.
Once you reach middle age you have to think about health and that high gear isnt good for the body.
