Not a film, but Lucas Black's New Orleans accent is the worst fricking thing I've ever heard in my life. Like nails on a chalkboard. That one is the worst for me, easily.

Worst - I’ll nominate Nicholas Cage for his Alabama accent in Con Air. I grew up in Alabama and have heard accents from every part of the state. None of them sounded even remotely like that.
That still never kept me from enjoying the movie though. It did give us this gif after all.

Worst: Robert Duvall's British accent in The 7% Solution
When I was a young teen I saw Joanne Woodward in Faulkner's Sound & The Fury I was very impressed, "Finally somebody doing a decent Southern accent". I didn't know she was from GA and went to LSU.
I like Mitchum's in Night Of The Hunter. Listen to the way he stresses certain words (in a way few other actors would), as in the jail cell scene: "Ah smuggled it in raht under the noses o' them guards!"
