Louisiana: Haynesville Golden Tornado with over 700 wins and 15 state titles.

Georgia: Valdosta Wildcats---started the program in 1913 and their overall record is 861-202-34
23 state championships
6 "national championships"(as mythical as they are for high school)
All of these have been in the highest classification in the state and they play in undoubtedly the toughest region in Georgia

East St. Louis is probably the winningest program in Illinois off the top of my head. They're always consistently good.
My dad went to Washington-Massillon in Ohio. 22 state titles and a few national titles I think. Pretty sure they're still second to Valdosta in wins nationally.

They haven't had a football program as long as Massillon but Cincinnati Moeller is still impressive.

In terms of state championships, here in Texas it's Celina (north of DFW) with eight (2007, 2005, 2001, 2000, 1999, 1998, 1995, and 1974; the 1974 was a co-championship while the others are outright).

I'm not sure about winning-est, but in terms of wins in Texas:
1 Highland Park (733)
2 Amarillo (720)
3 Plano (718)
Plano has the most state titles of these three. Due to new high schools being built, they and Amarillo aren't as dominant as olden times. Highland Park is a richie district and arm-twists the UIL to let them remain a class lower than the big boys since the 80's. So they have been and will continue to pull away every year.
Celina a 2A school does have more state titles than anyone, one more than Plano, but fewer wins.
