need rec for lipidologist or "up to date" cardiologist in BTR

I just got my self-order lab tests back from Ulta Lab [performed at Quest] and had some surprising numbers I need to get checked out with a professional.

Bad results:
ApoB: 95 mg/dl
LDL particle number: 1800 nmol/L
LDL small: 224 nmol/L
LDL C: 125 mg/dL
Total cholesterol: 230 [I know this isn't relevant much in modern science anymore]
lipoprotein (a): 126!!!!! [this is the first time I've
ever done this test]

good results:
triglycerides: 61
HDL: 90
LDL pattern: A
HS crp: 1.3 [not bad]
homocysteine: 5.2

Also of note, my omega 3 index is 6.8 which is good, but I want it to be higher. My omega6/omega3 ratio is 3.6 but I'd like it lower.

I'm really concerned about the lipoprotein (a). According to Peter Attia, I'm going to die any day now, but per Dr. Nadir Ali, it's ok.

And before you say "take a statin," I'm APOE4, and statins cross the blood brain barrier and deprive the brain of much-needed cholesterol which I need to help prevent/prolong alzheimers.

Regarding physical condition and diet, I'm female, 54, 5'11" and 140 lbs with low BMI, work out, bike, walk daily and eat a very low carb diet minus the emphasis on sat fats. Don't eat beef or fried foods -- maybe a few egg yolks a week, about three teaspoons of coconut oil a week to cook the eggs in, and daily tbsp of MCT oil powder. Just eat low carb Greek yogurt, blueberries, some nuts, green salads, chicken or salmon, veggies, protein smoothie with flaxseed, berries, kale and spinach every day. Maybe one drink a week.

If you are metabolically healthy, I wouldn't worry about having lipids that are a bit high.

Calcium score 0, no way I would take a statin.

Listen to Paul Saladino (carnivoreMD) on spotify, he addresses this very issue on his podcast.

Oh, and eat some beef and beef organs and get some good fat in your life.

I’ve stopped following Attia for the most part. I like him still but doesn’t sit right for some reason. I know that isn’t scientific. But neither is a lot of his clinical guesswork.

I’m not sure what else you’d really do here so go live your life and don’t add unnecessary stress to risk factors.

Yeah the anti-beef movement is absurd. Organ meet is one of the best things one can eat.

Other complete proteins like sardines too.

And whole eggs (pasture raised and eat the damned yolk. people).

OP what are you really looking to change on the numbers?

also are you lifting weights currently, as in a legit progressive overload lifting of weights?

Either way, I'd be more concerned with your ldl-p since it's pretty much in the stratosphere.

Being as he's a diplomat with the american board of clinical lipidology (board certified in lipidology), he's certainly more "up to date" than a general cardiologist, despite being an endocrinologist.

Thank you emanresu for the doc suggestion. Replying to other comments about sat fat and their benefits, diets high in sat fat are statistically linked to an increase in Alzheimer’s, specifically for people who are APOE4 which I am. I would imagine the sat fats I’ve been consuming on my modified keto [coco oil, mct oil, GF butter] are what have led to my elevated LDL levels.
